Approvals - must approve all items

When setting parameters for order approvals, there should be a box for "all items require approval".

We have some accounts that purchase $0 items but the account administrator still wants to see and approve these orders. The lowest you can set the Authorized Dollar Limit is $0.01 so these orders currently go through without approval.
